Telehealth Functional Fine Motor Assessment (FFMA)

This 6-page Microsoft Word document is a template that guides you through the completion of a preschool/school-age functional fine motor observational assessment. It is designed to allow easy administration in the telehealth or in person setting with very few items needed that can be found in the child's home. You can save the document for each student and edit and modify it easily while working with the student. The template contains links to videos that demonstrate some tasks, as well as pictures of 12 different pencil grasps to select from based on observations of the child's grasp. Included are clear descriptions of developmental age and grade expectations for informal observations based on existing research and literature.
